Moxibustion in pregnancy and for breech babies
What the 2023 Cochrane review found on moxibustion for breech presentation (BL67), its side effects and limits, why professional supervision matters, and why to check moxa ingredients in pregnancy.
Short answer: a 2023 Cochrane review found that moxibustion added to usual care probably reduces the number of babies still breech at birth, rated as moderate-certainty evidence. It probably does not reduce caesarean births, and more evidence is needed on side effects [1]. It is a “probably helps”, not a proven or guaranteed method.
Medical caution. Breech presentation needs assessment by a midwife or doctor. This page reports published evidence only; it does not recommend moxibustion or tell you how to do it. Do not start moxibustion in pregnancy without professional guidance.
What does the Cochrane review say about breech babies?
The review (Coyle, Smith & Peat, Cochrane Database of Systematic Reviews 2023, CD003928) included 13 trials with 2,181 women [1]. The method studied is moxibustion at the acupuncture point BL67 (Zhiyin), at the tip of the little toe [1].
| Outcome | Finding | Certainty |
|---|---|---|
| Baby still breech (non-cephalic) at birth, moxa + usual care vs usual care | Probably fewer (7 trials, 1,152 women) | Moderate |
| Same outcome, moxa vs sham moxa (both with usual care) | Probably fewer (1 trial, 272 women) | Moderate |
| Caesarean section | Probably little or no difference | Moderate |
| Need for external cephalic version (a doctor turning the baby) | Very uncertain | Low |
| Use of oxytocin | Probably reduced (one study) | Moderate |
| Side effects | More evidence needed | — |
The review also notes that starting before 37 weeks probably reduces the chance of the baby being head-up at birth, without reducing caesarean births [1].
Independent overviews reach the same overall view: breech presentation is the condition where moxibustion evidence is relatively clearest [2].
What side effects were reported?
In the breech trials, the most frequently reported side effects were increased fetal movements, uterine contractions, nausea, headache and burns from holding the stick too close to the skin [1]. A separate review of moxibustion case reports includes fetal distress and premature birth among reported adverse events [3].
Is moxibustion safe during pregnancy?
The sources do not support self-treatment without guidance:
- The Cochrane summary describes moxibustion that the woman or family members administer after being trained to do it safely [1].
- A moxa equipment maker advises people who are not licensed providers never to use moxa when pregnant or on pregnant women [4].
- Cleveland Clinic advises telling your acupuncturist if you are pregnant [5].
Women in parenting forums describe trying moxa alongside other approaches such as postural exercises, and many say they cannot tell which, if any, made the difference [6][7]. Individual stories of a baby turning, or not turning, are not evidence either way.
Can I use smokeless moxa for a breech baby?
Many UK buyers of smokeless sticks say they bought them for this purpose [7]. Two points matter:
- The trials do not tell us the moxa type. The Cochrane summary does not say whether smokeless or smoky moxa was used, and smokeless sticks differ in heat and infrared output [8]. Results may not transfer directly.
- Check the ingredients. Smokeless sticks can contain herbs besides mugwort [9][10]. One buyer reported a stick sold as “pure moxa” that contained motherwort (Yi Mu Cao, Leonurus), which they had understood was not suitable in pregnancy [7]. Ask the seller for a full ingredient list, and ask your midwife or doctor before use.
Does moxa help you get pregnant?
Search data show strong interest in “moxa for fertility”, “moxa for IVF” and, in Arabic, الموكسا للحمل [11]. We found no reliable review establishing that moxibustion improves fertility, so we make no claim. Fertility concerns should go to a doctor.

Can I use smokeless moxa for a breech baby?
The Cochrane review does not report whether trials used smokeless moxa, so evidence from smoky moxa may not transfer directly. Check ingredients too: some sticks contain herbs other than mugwort.
