Moxibustion and cupping: differences and combined use

Moxibustion (heat from burning moxa) and cupping (suction cups, including Arabic hijama) are different therapies often combined in Chinese practice. What trials report on combinations for shingles, functional diarrhea, asthma and athletic fatigue.

Short answer: moxibustion applies heat from burning moxa [1]. Cupping, including the Arabic tradition of hijama (الحجامة), applies suction with cups. Chinese practice sometimes combines them, and Arabic searches link the two, for example “فوائد حجامة الموكسا” [2].

Studies of combined treatment

Condition Combination Source Reported finding Caveat
Post-herpetic neuralgia Acupuncture, moxibustion and cupping Meta-analysis, 2021 [3] Favorable pooled outcomes in the included comparisons Multiple components; another moxa review calls evidence inconclusive [4]
Acute herpes zoster Joss-stick moxibustion, pricking and cupping Randomized study, 2017 [5] Reported pain and lesion outcomes compared with a medication regimen One study; cannot isolate one component
Functional diarrhea Cupping moxibustion added to oral montmorillonite Randomized study of 51 patients, 2022 [6] Reported symptom and other measures Both groups received medication; one study
Cold-type asthma Blistering cupping plus Thunder-fire moxibustion added to a standard inhaler Randomized study of 76 patients, 2023 [7] Reported symptom and lung-function measures Combination added to conventional care; not a moxa-alone comparison
Athletic fatigue Mild moxibustion plus cupping Study, 2007 [8] Serum creatine kinase response after training Combined treatment and a laboratory marker

Safety

Cupping and moxibustion have different risk pathways. A case-report review documents moxa-related burns [9]. The US National Center for Complementary and Integrative Health distinguishes dry cupping, which does not pierce skin, from wet cupping, which does; it also describes possible skin and infection harms [10]. Published home-moxa instructions advise against application on broken skin [11]. The earlier version of this page cited a moxa-smoke exposure paper for wet-cupping skin injury; that citation was unrelated and has been removed.

The key interpretation: a combination is not one ingredient

The trials in the table test treatment packages. The asthma trial gave its control group a standard inhaler and added blistering cupping plus Thunder-fire moxibustion in the other group [7]. The functional-diarrhea study used oral montmorillonite in both groups and added cupping moxibustion to one [6]. The acute-zoster study combined joss-stick moxibustion, pricking and cupping and compared that package with a medication regimen [5]. None answers the same question as “Does a smokeless charcoal stick by itself treat this condition?”

The meta-analysis on post-herpetic neuralgia pools studies of acupuncture, moxibustion and cupping as a combination [3]. A separate systematic review of moxibustion for the same condition described the evidence as inconclusive [4]. Their different intervention definitions and included studies should be checked before treating the conclusions as a contradiction. A narrative or pooled result also cannot establish that a particular retail kit or self-treatment method has been evaluated.

For a reader comparing methods

Ask first whether the question concerns heat, suction, skin piercing or a package that includes several of them. Then identify who administered the treatment and what the control group received. Dry and wet cupping are not the same procedure [10], and a study of combined treatment does not tell the buyer how much of an observed difference came from moxa. For a product discussion, keep moxa material, cup type, instructions and warnings as separate items in the brief. The sports evidence page shows another example of why combined interventions cannot be attributed to moxa alone.

FAQ: related questions

What is the difference between moxibustion and cupping?

Moxibustion applies heat by burning moxa on or near the skin; cupping applies suction with cups. In Chinese practice they are sometimes combined in one treatment, and Arabic searchers often ask about 'moxa cupping' because hijama is familiar.

Is cupping with moxibustion effective?

Some trials report favorable outcomes for specific combined protocols, but they do not isolate moxibustion from cupping, acupuncture or standard medication. Population, comparator and study quality must be read before drawing a treatment conclusion.
