Moxibustion for IBS and digestive complaints: what the research says
Sourced summary of an umbrella review, meta-analyses and trials on moxibustion for irritable bowel syndrome (especially diarrhea-predominant IBS) and constipation, with evidence-quality caveats. No treatment claims.
Short answer: digestive complaints are among the oldest traditional uses of moxa, and diarrhea is listed among moxibustion’s most common indications in Chinese practice [1]. For irritable bowel syndrome (IBS), an umbrella review found that most systematic reviews interpreted their results as beneficial, but rated the methodological and evidence quality as needing improvement [2].
Evidence summary only. Persistent changes in bowel habit, blood in stool, weight loss or night symptoms need medical assessment. Moxa Sourcing makes no treatment claims.
Irritable bowel syndrome
| Source | Scope | Reported finding | Caveat |
|---|---|---|---|
| Umbrella review, 2024 [2] | Systematic reviews of acupuncture and moxibustion for IBS | Most reviews suggest benefit | Quality of reviews and evidence needs to improve |
| Meta-analysis, 2022 [3] | Moxibustion RCTs for diarrhea-predominant IBS (IBS-D) | “Preliminary” evidence of effectiveness | Rigorous, large RCTs needed |
| Meta-analysis, 2022 [4] | Acupuncture and/or moxibustion | Benefit for symptom severity, abdominal pain and quality of life | Sham-controlled moxibustion results show “great heterogeneity” |
| Network meta-analysis, 2022 [5] | Treatments for IBS-D | Acupuncture plus “sandwiched” (indirect) moxibustion ranked highest | Limited number and quality of studies |
| Meta-analysis, 2013 [6] | Moxibustion for IBS | “May provide benefit” | Relatively high risk of bias |
| RCT, 2022 [7] | Mild moxibustion vs placebo moxibustion for IBS-D | Mild moxibustion more effective, lasting up to 12 weeks | Retrospectively registered |
The trial against placebo moxibustion [7] matters because it is one of the few designs that try to separate moxa’s specific effect from expectation. Even so, a meta-analysis across sham-controlled trials found the moxibustion results highly inconsistent [4].
Anxiety and depression in IBS
A Chinese meta-analysis reported improvements in anxiety and depression scores with some forms of acupuncture and moxibustion in people with IBS, and said other forms need more study [8].
Constipation
Evidence on constipation is mostly about acupuncture and moxibustion combined, often in specific groups. For example, a network meta-analysis on constipation after stroke ranked acupuncture combined with medication highest [9]. Earlier overviews also list constipation among conditions with mixed or contradictory evidence [10].
Laboratory research
Animal studies of IBS-D report changes in gut sensitivity, inflammation and immune balance after moxibustion. The reviewing authors say these findings need validation because of study quality [11]. Animal results do not show an effect in people.
Methods used in digestive trials
Trials commonly apply moxa over the abdomen, around the navel and on points on the legs. Methods include mild (suspended) stick moxibustion, moxa boxes, and indirect moxibustion on ginger, salt or herbal cakes [5][12]. See Ginger, garlic and salt moxibustion and Moxibustion techniques.

FAQ: related questions
Does moxibustion help IBS?
An umbrella review found that most systematic reviews interpreted their findings as showing benefit of acupuncture and moxibustion for IBS, but said the methodological, reporting and evidence quality of those reviews needs to improve.
Has moxibustion been compared with a placebo for IBS?
Yes. A randomized trial of mild moxibustion for diarrhea-predominant IBS reported better results than placebo moxibustion for up to 12 weeks; a meta-analysis noted large heterogeneity in moxibustion's sham-controlled results.
